Thank you for your interest in University Care Advantage (HMO SNP)!

University Care Advantage (UCA) is specially designed just for people who are entitled to Arizona State Medicaid (AHCCCS) and also qualify for Medicare coverage. 

If you have UCA, you have a special enrollment period and can enroll any month you choose.

Here are some key qualifying points you must check:
- Are you entitled to Medicare Part A and enrolled in Medicare Part B?
- Do you have Arizona State Medicaid (AHCCCS)?
- Do you live in the service area of the plan? (Must live in Cochise, Gila, Graham, Greenlee, La Paz,   Pima, Pinal, Santa Cruz, Yavapai or Yuma County)

Individuals with End-Stage Renal Disease generally are not eligible to enroll with us, unless they are members of our organization and have been since their dialysis began.

*Eligible beneficiaries must use network pharmacies to access their prescription drug benefit except under non-routine circumstances, and quantity limitations and restrictions may apply.

How to Enroll

You can call our enrollment line at 1-877-874-3938, 8:00 a.m. - 8:00 p.m., 7 days a week and speak to one of our experienced agent representatives. Calls to this number will direct you to a licensed insurance agent. Our licensed agents will help you understand how our plan works and explain our benefit coverage.

You can fill out the Enrollment Form and mail it back to us. (See below for the Enrollment Form)
